Once you contact us, we immediately start investigating your plan. To speed up this process, we ask our clients to fax or email us the Patient Registration Form as soon as possible. It allows us to jumpstart verifying your insurance.
We receive weekly updates on any governmental policy changes and regularly attend Medicare/Medicaid seminars.
Prior to your first visit, we suggest you:
- Fill out Patient Registration Form. This form should be completed prior to your first visit and may be faxed or emailed to our office.
- If you are a member of an HMO, an authorization from your primary physician is required. Please call our office and ask to speak with an Insurance Specialist, just let them know your authorization number prior to your visit.
